Section 20 CSR 2150-3.163 - Physical Therapist Assistant Inactive LicensePURPOSE: This rule provides the requirements physical therapist assistants must follow to request inactive status.
(1) Licensees shall make application for an inactive license upon a form prepared by the board.(2) No application will be considered unless fully and completely made out on the specified form and properly attested.(3) All applications shall be sent to the Missouri State Board of Registration for the Healing Arts, PO Box 4, Jefferson City, MO 65102.(4) To be eligible for an inactive license, a licensee must hold a current and active license to practice as a physical therapist assistant in the state of Missouri and shall not be under investigation by the board or involved in pending disciplinary proceedings.
