Rule 11-4-7.8 - Method of Reporting Changes in Circumstances

Changes in circumstances shall be reported on amendment forms provided by the Department. The amendment form(s) shall be signed and sworn to or affirmed by the same person(s) and in the same manner as authorized by Rule 7.6 above for the disclosure statement.
