Section R. 322.1009 - Permit application proceduresRule 9.
(1) An application for a permit shall be made on a form as prescribed and provided by the department. Application forms may be obtained from the department or from any designated field office of the department.(2) An application for a permit shall not be considered administratively complete until all information requested on the application form, the application fee, and any other information requested by the department have been received by the department. After receipt of an otherwise complete application, the department may request such additional information, environmental assessments, records, or documents as are determined to be necessary to make a decision to grant or deny a permit. The department shall notify the applicant in writing when the application is administratively complete.(3) If an applicant fails to respond to any written inquiry or request from the department within 30 days, the application shall be denied without prejudice and the file shall be closed.(4) For upland channels, a fee of not less than $50.00, payable to: "State of Michigan," shall accompany the application.Mich.
