Section R. 285.811.4 - Application and approval of allotments

Rule 4.

(1) The president and secretary of an association that requests state allotments for the payment of premiums shall submit to the department a properly completed application by the date specified by the department on the application. The department shall give written notice if the application is denied.

(2) Any change to the original application shall be submitted in writing to the department.
