Section 137-23-1.06 - Special Application Procedure Form

An application form for waiver of the Camp Trip Leader Safety Course is to be filled out by each applicant, and applicants Camp Director. The following information is required; documentation to accompany application of all certificates required in prerequisites.

A. Name and legal residence of applicant.
B. Date of birth of applicant.
C. Name and address of applicants sponsoring Camp.
D. Signature of Camp Director attesting his belief the applicant is qualified.
E. Signature of applicant attesting all information to be true and correct.
