Section I-9441 - Delivery of ServiceA. There shall be a complete history and physical work-up in the chart of every patient prior to surgery, except in emergency surgery. If the history and physical has been dictated, but not yet recorded in the patient's chart, there shall be a statement to that effect and an admission note in the chart by the practitioner who admitted the patient.B. A properly executed informed consent form for the procedure shall be in the patient's chart before surgery, except in emergencies. The consent form shall contain at least the following: 2. hospital and patient identification number;3. name of the procedure(s) or operation;4. the reasonably foreseeable risks and benefits involved;5. name of the practitioner(s);6. signature of the patient or legal guardian;7. date and time the consent is obtained; and8. date, time, and signature of the person witnessing the patient or the patient's legal representative sign the consent form.La.
