Section I-5361 - Personnel RecordsA. A CRC shall maintain a personnel file for each employee and direct care staff member in the center. Each record shall contain: 1. the application for employment and/or resume, including contact information and employment history for the preceding five years, if applicable;2. reference letters from former employer(s) and personal references or written documentation based on telephone contact with such references;3. any required medical examinations or health screens;4. evidence of current applicable professional credentials/certifications according to state law or regulations;5. annual performance evaluations to include evidence of competency in performing assigned tasks;6. personnel actions, other appropriate materials, reports and notes relating to the individual's employment;7. the staff member's starting and termination dates;8. proof of orientation, training and in-services;9. results of criminal background checks, if required;10. job descriptions and performance expectations;11. a signed attestation annually by each member of the direct care staff indicating that he/she has not been convicted of or pled guilty or nolo contendere to a crime, other than traffic violations; and12. written confidentiality agreement signed by the personnel every twelve months.B.
