Rule 641-3.17 - Eligibility criteria

The enrollment process to determine eligibility for services under this program includes the following requirements:

(1) Age. Individuals are eligible from birth through 20 years of age.

(2) Residency. Individuals must currently reside in Iowa.

(3) The applicant must not be eligible for hearing aids or audiologic services under Title XIX or HAWK-I.
