Rule 481-507.20 - Speech pathology and audiology license fees

All fees are nonrefundable.

(1) License fee for license to practice speech pathology or audiology, temporary clinical license, license by endorsement, or license by reciprocity is $120.

(2) Biennial license renewal fee for each biennium is $96.

(3) Late fee for failure to renew before expiration is $60.

(4) Reactivation fee is $156.

(5) Duplicate or reissued license certificate fee is $20.

(6) Verification of license fee is $20.

(7) Returned check fee is $25.

(8) Disciplinary hearing fee is a maximum of $75.

(9) Temporary clinical license renewal fee is $60.

(10) Temporary permit fee is $30.

This rule is intended to implement Iowa Code chapters 17A, 147 and 272C.
