Section 856 IAC 1-23-1 - Dispensing of dangerous drugsAuthority: IC 25-26-13-4

Sec. 1.
In the sale or dispensing of any prescription drug or narcotic, the pharmacist shall be required to affix to the immediate container in which such prescription drug or narcotic is delivered a label bearing the following information:
(1) The name, address, and telephone number of the establishment from which such drug was sold.(2) The date on which the prescription for such drug was filled.(3) The number of such prescription as filed in the prescription files of the pharmacy where the prescription was filled.(4) The name of the practitioner who prescribed such drug.(5) The name of the patient, and if such drug was prescribed for an animal, a statement of the species of the animal and the owner's name.(6) The directions for use of the drug as contained in the prescription.(7) The name of the drug (trade or generic, or both) in compliance with the Generic Drug Law found in IC 16-42-22.
