Sec. 28.
(a) An applicant for a child care home license shall do the following: (1) Attend an orientation training arranged or approved by the CDFC.(2) Comply with application procedures required by this rule.(3) Submit to the COFC a completed, signed application for a child care home license, including attachments required to demonstrate compliance with this rule.(4) Submit a statement attesting that the applicant has not been:(A) convicted of a felony or a misdemeanor relating to the health and safety of children; and(B) charged with: (ii) a misdemeanor relating to the health and safety of children; during the pendency of the application.(5) Within thirty (30) days of application, submit a written medical statement, including proof of a Mantoux tuberculin test or chest x-ray, signed by a physician or a certified nurse practitioner.
