Section 460 IAC 6-5-10 - Enhanced dental services provider qualifications

Sec. 10.

(a) To be approved to provide enhanced dental services, an applicant shall be a dentist licensed under IC 25-14.
(b) For an entity to be approved to provide enhanced dental services, the entity shall certify that, if approved, the entity will provide enhanced dental services using only persons who meet the qualifications set out in this section.
