Section 410 IAC 18-1-4 - Authorization of local boards for use of provisional notification

Sec. 4.

Local health departments shall:

(1) make application for use of a provisional notification of death; and

(2) have unrevoked written authorization from the state department prior to the use of any provisional notification of death.
