Section 405 IAC 5-2-22 - "Provider agreement" defined

Sec. 22.

"Provider agreement" means a contract or certification agreement between a provider and the office setting out the terms and conditions of a provider's participation in Medicaid, which must be signed by such provider prior to any reimbursement for providing covered services to members.
