Section 16.06.13.150 - APPLICATION FOR SERVICES

An application must be completed and signed by one (1) of the following individuals on behalf of the eligible child for emergency assistance to be given:

01.A Parent or Parents.

02.An Adult Relative. An adult relative may sign on behalf of the child, when the child is residing with them and they are responsible for the child's care.
