Section 8-54-10.3 - Demand for a hearing

Any person whose application is denied, or a licensee or permittee who is to be disciplined by the board, shall be entitled to a hearing if a demand for hearing is filed with the board's office within sixty(60)days of the date of the letter informing the applicant of the denial or the licensee or permittee of the intent to discipline, respectively. The demand for hearing shall contain concise statements of:

(1) the legal authority under which the hearing is to be held;
(2) the denial or matter that is being contested by the petitioner;
(3) the basic facts and issues raised; and
(4) the relief to which the petitioner deems itself entitled.
