The fees for the board of physical therapy shall be as provided in this section:
| (1) Application fee | $ 50 |
| (2) Examination and other examination related fees as determined by agreement between the board and a professional testing service | |
| (3) Original license fee | $ 30 |
| (4) License fees: | |
| (A) First year of the biennium (original license fee, plus one-half of the biennial renewal fee) | $ 80 |
| (B) Second year of the biennium (original license fee) | $ 30 |
| (5) Temporary license fee | $ 30 |
| (6) Biennial renewal fee | $ 100 |
| (7) Inactive and reactivation fee | $ 10 |
| (8) Restoration for failure to renew license shall be the payment of the delinquent fees plus an amount equal to fifty per cent of the delinquent fee. | |
| (9) Compliance resolution fund: | |
| (A) Compliance resolution fund fee | $ 35 |
| (B) Annual compliance resolution fund fee | $ 35 |
| (C) Biennial compliance resolution fund fee | $ 70 |
