The fees for the board of dental examiners shall be as provided in this section:
| (1) Application fee | $ 30 |
| (2) Examination and reexamination fee | $ 200 |
| (3) Examination handscore fee | $ 20 |
| (4) Examination review fee | $ 25 |
| (5) Original license fee | $ 40 |
| (6) License fees: | |
| (A) First year of the biennium (original license fee, plus one-half of the biennial renewal fee) | $ 60 |
| (B) Second year of the biennium (original license fee) | $ 40 |
| (7) Application fee for certification in the administration of intra-oral infiltration local anesthesia and intra-oral block anesthesia: | |
| (A) Intra-oral infiltration local anesthesia | $ 10 |
| (B) Intra-oral block anesthesia | $ 25 |
| (8) Biennial renewal fee | $ 40 |
| (9) Restoration fee | $ 50 |
| (10) Compliance resolution fund: | |
| (A) Compliance resolution fund fee | $ 35 |
| (B) Annual compliance resolution fund fee | $ 35 |
| (C) Biennial compliance resolution fund fee | $ 70 |
