Section 16-110-6 - Identification of personnel

(a) A licensed physical therapist shall wear a conspicuously placed name tag stating, at a minimum, the person's name and the title "physical therapist" or "PT".

(b) The supervising physical therapist shall ensure that all support or auxiliary personnel wear a conspicuously placed name tag identifying the individual's name and professional designation at the time of care.
