Rule 560-10-22-.04 - Certified Firefighter License Plates(1) Application for a Certified Firefighter license plate must be submitter to the Motor Vehicle Division or county tag agent on form MV-9F accompanied by a manufacturing fee of $25.00. The firefighter's social security number and the name of the legally organized fire department or volunteer fire department with which the applicant is affiliated must be shown in the designated blocks.(2) Application must be accompanied by certification from the applicant's fire chief stating their eligibility.(3) Only one Certified Firefighter license plate may be issued to a qualified applicant.(4) Applications which are denied will be returned to the applicant and the manufacturing fee refunded.Ga.
