License fees for home care agencies shall be based upon an annual census of admissions of patients served in the District of Columbia. The fees shall be as follows:
(a) | Initial Application | $ 600 |
(b) | 1 - 150 Patients | |
Annual Fee | $ 400 | |
Late Fee | $ 100 | |
(c) | 151 - 750 Patients | |
Annual Fee | $ 700 | |
Late Fee | $ 100 | |
(d) | 751 - 1250 Patients | |
Annual Fee | $ 1100 | |
Late Fee | $ 100 | |
(e) | 1251 or More Patients | |
Annual Fee | $1300 | |
Late Fee | $ 100 | |
(f) | Duplicate of License | $ 50 |
(g) | Certification of Validity of | |
License | $ 50 |
Time periods for the submission of applications and fee payments shall be in accordance with Chapter 31 of Title 22 of the District of Columbia Municipal Regulations.
