Rule 17-5508 - REINSTATEMENT OF AN EXPIRED LICENSE
5508.1

An applicant for reinstatement of a license shall:

(a) Submit proof as set forth in § 5510 of having completed eighteen (18) hours of continuing education in the applicant's current area of practice in the two years immediately preceding the application date; and
(b) Meet any other requirements that the Board may set forth to determine whether the license should be reinstated.
5508.2

An applicant for reinstatement of a license shall submit the required documents and completed forms required by the Board and pay the required fees.

5508.3

The Board shall not reinstate the license of an applicant who fails to apply for reinstatement of the license within five (5) years after the license expires. The applicant may become licensed by applying for and meeting the requirement for obtaining an initial license which shall include licensure by the re-entry program.
