A person may apply for reinstatement of an expired license any time within five (5) years of the date that the license expired by submitting a completed reinstatement application and paying the appropriate fees.
The Board shall approve an application for reinstatement of an expired license if the applicant complies with current requirements for license renewal and submits satisfactory evidence of compliance with any continuing education requirements established by the Board in this chapter.
The Board shall not reinstate the license of a person who fails to apply for reinstatement within five (5) years after the last valid license held by the person has expired. The applicant for reinstatement may be required to demonstrate to the Board that he or she has maintained the required minimum level of professional competence in a manner acceptable to the Board.
