Section 21a-1-15 - Form, contents and filing of applicationsAll applications shall include
(i) the name and address of the applicant;(ii) the name and address of the applicant's counsel, agent, or other representative, if any;(iii) the purpose for which the application is made;(iv) any statutes and rules which support the application;(v) a complete and concise description of the activities, facilities, projects, or other actions for which the license, permit or registration is sought;(vi) any other information which the department may require; and(vii) any additional information which the applicant considers relevant. Applications shall be addressed to the appropriate division of the department and shall be sent by mail or hand-delivered during normal business hours.
