Section 19a-9-10 - Petition contents(a) A petition shall be in writing and shall contain the following: (1) the full name and address of the petitioner;(2) the full name and address of the respondent;(3) a plain and simple statement of the facts, events or actions on which the claim is based;(4) the date and approximate time of the alleged violation; and,(5) the location where the alleged violation occurred.(b) A petition shall not be deemed defective solely because of the absence of one or more of the items contained in subsection (a) of this section.Conn.
