Section 14.01 - Petitions(1) All petitions filed under 456 CMR 14.00 shall be in the form prescribed by the Department.(2) All petitions filed under 456 CMR 14.00 shall be in writing and shall contain a declaration by the person signing them, under the penalties of perjury, that the contents are true and correct to the best of the signatory's knowledge or belief.Amended by Mass Register Issue 1322, eff. 9/23/2016.
