Section 3.03 - Full License Application(1) An application for a license as a physician assistant shall be made on a form provided by the Board. The application form shall be filled out completely and shall be accompanied by the following:(a) An authorization signed by the applicant, which authorizes the NCCPA to release such information as the Board may deem necessary to verify that the applicant has passed the NCCPA certifying examination;(b) An official transcript, or other official documentation satisfactory to the Board, verifying that the applicant has graduated from a physician assistant training program that was an Approved Program on the date of graduation;(c) An official transcript, or other official documentation satisfactory to the Board, verifying that the applicant meets the educational requirements specified in 263 CMR 3.02;(d) Documentation satisfactory to the Board demonstrating completion of training required pursuant to M.G.L. c. 94C, § 18(e);(e) Any and all additional documentation which may be requested in writing by the Board; and(f) A valid form of payment made payable to the Commonwealth of Massachusetts for the amount of any application and license fees, as determined by the Executive Office of Administration and Finance pursuant to M.G.L. c. 7, § 3B, unless waived in accordance with M.G.L. c. 112, § 1B.(2) All fees submitted in conjunction with an application for license are non-refundable.Amended by Mass Register Issue 1335, eff. 3/24/2017.
