Section 8.16 - Change of Name, Ownership, Location, or Services(1) The provider shall provide prior notification in writing to the Department of any change in its name or in its ownership.(2) The provider shall notify the Department in writing of any change in its executive director of the services or supports.(3) The provider shall notify the Department in writing of any changes in the physical location of the service or support, or of any other changes which place the provider out of compliance with any regulation under 115 CMR, within ten days of the change.(4) The failure of a provider to notify the Department of any change of name, ownership, location, or services and supports shall be grounds for suspension or termination of the license.Adopted by Mass Register Issue 1317, eff. 7/15/2016.
