The individual has the following responsibilities in filing an appeal.
(a) All grounds shall be included in the same appeal.(b) Specific grounds shall be stated, and all necessary documents and information shall be attached to the application.(c) The decision desired shall be stated.(d) Assistance in preparing the appeal may be sought from staff or others.(e) The appeal shall be filed with the Narcotic Addict Evaluation Authority within 30 days of the date the action was taken.
