Section 2177.3 - Filing and Service Requirements(a) An original and one copy of the Request for Hearing and all documents provided must be sent to the California Department of Insurance, Administrative Hearing Bureau, 1901 Harrison Street, 3rd Floor Mailroom, Oakland, CA 94612.(b) All required documents and copies must be delivered to the Administrative Hearing Bureau either by personal delivery or U.S. Postal Service Return Receipt Requested delivery.Cal.
