Section 58-1-301.7 - Change of information(1)(a) An applicant, licensee, or certificate holder shall notify the division within 10 business days of a change in mailing address or email address.(b) When providing a mailing address, the individual may provide a post office box or other mail drop location.(c) In addition to providing a mailing address, an applicant, licensee, or certificate holder shall provide to the division, in a form approved by the division, an email address.(2) An applicant, licensee, or certificate holder is considered to have received a notification that has been sent to the most recent: (a) mailing address provided to the division by the applicant, licensee, or certificate holder; or(b) email address furnished to the division by the applicant, licensee, or certificate holder.Amended by Chapter 339, 2020 General Session ,§ 5, eff. 5/12/2020.Amended by Chapter 262, 2013 General Session ,§ 4, eff. 5/14/2013.Enacted by Chapter 367, 2011, 2011 General Session
