Section 686A.360 - Application for license: Form; fees; bond; required documents; provision of material changes to Commissioner1. An application for a license to engage in the business of a company must be filed with the Commissioner on a form prescribed by the Commissioner and must include: (a) A nonrefundable fee for application and for investigation of the applicant of $500 and, in addition to any other fee or charge, all applicable fees required pursuant to NRS 680C.110;(b) A surety bond which complies with NRS 679B.175 and is in an amount determined by the Commissioner;(c) A current certified financial statement or another financial statement if individually approved by the Commissioner;(d) An appointment of the Commissioner and the successors in office of the Commissioner as the applicant's attorney to receive service of process; and(e) If the applicant is a corporation, a copy of its articles of incorporation.2.
