§ 714h. Evidence of service

The joint presentation of the following documents shall constitute evidence of service:

(1) Military identification.

(2) Document awarding any decoration or any order delivered, if applicable, for having rendered such service or for having been in active service during the concerning dates.

(3) Form DD-214 (Department of Defense Form 214) or the NG-22 (National Guard Form 22).

(4) Commander’s Verification of Deployment.

(5) A copy of the Official Personnel Orders.

(6) Permanent Change of Station Orders (PCS Orders).

A letter of recommendation certified by the components and corps listed in subsections (a), (f), (g), (h), and (n) of § 714-1 of this title indicating that the service member has in fact met the requirements to avail him/herself of the provisions of any of the sections of this chapter shall also be accepted as evidence of eligibility.
