Rule 3 - Notice of Appeal
(a) An appeal shall commence by filing a written notice of appeal with the trial court. A notice of appeal or cross-appeal filed by mail must be received by the trial court within the time provided for in Rule 4 of these rules.
(b) The notice of appeal or cross-appeal shall identify the order, judgment, or ruling that is appealed.
(c) When a party appeals or cross-appeals, the trial court shall send a copy of the notice of appeal or cross-appeal to the opposing side(s). The notice of appeal or cross-appeal shall state all parties' or counsels' current mailing addresses and phone numbers. Such addresses may be used for further notices or proceedings unless the trial court is notified otherwise in writing.
(d) A party against whom a default judgment has been rendered must first move in the trial court to set aside that judgment. The party may then take an appeal from the order denying the motion.
