Section 7-25 - Return of Certificate or Permit

Any certificate or permit that has been revoked or suspended shall be returned to the Board office with fifteen (15) business days of receipt of notification of suspension or revocation. If a certificate has been lost or destroyed, the holder shall submit an Affidavit of Lost/Destroyed CPA Certificate to the Board's office.
