The forms prescribed by s. 409.521, Stats. shall be accepted by the filing office. Forms approved by the International Association of Corporate Administrators and forms approved by the department shall be accepted. When filing a correction statement, the "UCC correction statement" form, UCC-5, shall be used. Other forms shall not be accepted by the filing office.
