Section 354.1091 - Additional Claim Information Requirements

In addition to the general requirements in § 29.1 of this title (relating to Claim Information Requirements), the following information is required on claims for laboratory and x-ray services and radiation therapy; the name, address, and appropriate identification number (if known) of ordering physician.
