Section 5:04:08:01 - Application for certificate of authorization to provide postsecondary education

The application for certificate of authorization to provide postsecondary education, as prescribed by the Secretary of State, shall contain the following fields:

(1) Name of institution;

(2) Address of institution;

(3) Contact person for institution;

(4) Addresses of other locations where institution provides classes;

(5) Accrediting information; and

(6) Institution classification.
