Section 3800.184 - Medication log(a) A medication log shall be kept to include the following for each child: (1) A list of prescription medications.(2) The prescribed dosage.(3) Possible side effects.(4) Contraindicated medications.(5) Specific administration instructions, if applicable.(6) The name of the prescribing physician.(b) For each prescription and over-the-counter medication including insulin administered or self-administered, documentation in the log shall include the medication that was administered, dosage, date, time and the name of the person who administered or self-administered the medication.(c) The information in subsection (b) shall be logged at the same time each dosage of medication is administered or self-administered.
