The following is the schedule of fees charged by the Board:

Biennial renewal of nursing home administrators license... $297

License application fee... $40

Temporary permit fee... $145

Certification of examination scores... $25

Verification of licensure or temporary permit... $15

Continuing education provider application fee... $40

Continuing education program application fee per clock hour... $15

Continuing education individual program appli- cation fee... $20
