Section 21.232 - Records required to be filed in the Board office(a) An annual report of the practical nursing program shall be sent to the Board using the form supplied by the Board.(b) Upon completion of the entire program a transcript or photocopy of the final record of the student shall be submitted along with the application for admission to the State Board Test Pool Examination. The transcript shall bear the impression of the school seal and signature of the nurse in charge of the program or authorized representative.(c) A notarized list of nurses employed in the program shall be submitted by January 15 of each year. This list shall include the full name including maiden name, current address, license number and date of original employment in this Commonwealth.The provisions of this §21.232 amended July 1, 1983, effective 7/2/1983, 13 Pa.B. 2061.The provisions of this §21.232 issued under section 506 of The Administrative Code of 1929 (71 P.S. § 186); and section 8 of the Practical Nurse Law (63 P.S. § 658).
