Section 365:25-5-31 - Information to be included on applications

(a) Original and renewal applications shall include the applicant's:

(1) residence address,

(2) business address,

(3) mailing address,

(4) e-mail address ,

(5) telephone number, and

(6) legal name.

(b) An applicant's business address and mailing address shall be in the same county.
