Section 365:15-1-10 - Policy of insurance shall include insurer's complete name and street address and insurance claim warning

(a)Insurer's name and address required on policy.

Every policy of insurance issued or delivered in this State shall specify, on its face, or by endorsement, or rider attached to the policy:

(1) the complete name of the insurer;

(2) the complete street address of the home or principal office of the insurer.

(b)Insurance claim warning required. Every insurance claim form and insurance policy issued or delivered in this State shall specify on its face, or application, or by endorsement, or rider attached to the policy, printed in ten point or larger, the warning statement required in 36 O.S. § 3613.1.
