Section 4901:2-6-04 - [Rescinded] Supplementation of information(A) Upon a written request from the staff, an applicant shall provide any additional information necessary for the processing of a uniform application within fifteen days, unless otherwise agreed to by the applicant and the staff.(B) A carrier that has filed part III of the uniform application shall report to the commission, in writing, any changes to the information provided in the filing within ninety days after the date of such change.
