Section 645F.805 - Complaint alleging violation1. A person may file with the Commissioner a complaint alleging that another person has violated a provision of the Nevada Covered Service Provider Regulations or chapter 645F of NRS or an order of the Commissioner.2. A complaint filed pursuant to this section must:(b) Be signed by the person filing the complaint or the authorized representative of the person filing the complaint;(c) Contain an address and a telephone number for the person filing the complaint or the authorized representative of the person filing the complaint;(d) Describe the nature of the alleged violation in as much detail as possible;(e) Include as exhibits copies of all documentation supporting the complaint; and(f) Include any other information or supporting materials required by the Commissioner.Nev.
