Section 604A.050 - Application for licenseAn application for a license must, in addition to the requirements set forth in NRS 604A.600, include:
1. The name under which the applicant proposes to conduct his business.2. A financial statement for the applicant's most recent fiscal year or a copy of the applicant's federal income tax return for each of the preceding 2 years, or both.3. For each person who owns at least 25 percent of the voting stock, partnership or member interests of the business of the applicant, a record of the personal history of the person. The record must be submitted on a form provided by the Commissioner.Nev. Admin. Code § 604A.050
