Section 472-3-002 - Contracts

Clients may be required to contract with the approved provider(s) they will be using. Form DPF-3, "Disabled Persons and Family Support Provider Contract," may be used. The client shall submit a copy of the signed contract to the Department before Department funds may be paid to that provider.
