Rule 37.12.311 - DURATION OF LICENSE(1) A license granted to an in-state laboratory is for a three year period provided: (a) all of the requirements of this subchapter continue to be met for the type of license granted;(b) the laboratory completes annual questionnaires from the department designed to update the department on personnel and procedures; and(c) the laboratory remits to the department the appropriate annual licensure fee and any other fees due pursuant to ARM 37.12.310.(2) A reciprocal license granted to an out-of-state laboratory is for the same period as the license, certification or other approval granted by the approving authority, provided that: (a) all of the requirements of ARM 37.12.313 continue to be met;(b) the laboratory completes annual questionnaires designed to update the department on personnel and procedures; and(c) the laboratory remits to the department the appropriate annual licensure fee or fees due pursuant to ARM 37.12.310.Mont.
