Section V-17702 - Application ProcedureA. An entity seeking approval as a board approved doula training organization shall submit an application in a format or utilizing a form prescribed by the board. The application, instructions, and address or method of submitting an application are available on the board's webpage, located or linked to on the website of the Department of Health. The board shall have the discretion to require use of an online application submission system.B. An application for registration shall include proof, documented in a manner and form satisfactory to the board, that the applying entity meets the qualifications for approval set forth in this Chapter.C. The board may refuse to consider any application which is not complete in every detail, including submission of every document or type of documentation required by this Chapter. The board may, in its sole discretion, require a more detailed or complete response to any request for information set forth in the application form or this Chapter as a condition of consideration of an application.La.
