Section XV-474 - Records of Prior Occupational DoseA. The licensee or registrant shall retain the records of prior occupational dose and exposure history as specified in LAC 33:XV.414 on department Form DRC-4 or equivalent until the department terminates each pertinent license or registration requiring this record. The licensee or registrant shall retain records used in preparing Form DRC-4 or equivalent for three years after the record is made.B. Upon termination of the license or registration, the licensee or registrant shall permanently store records on department Form DRC-4 or equivalent or shall make provision with the department for transfer to the department.La.
